Errors in orthodontic treatment

My proposal is to run an anonymous survey via the British Orthodontic Society office to ask members if they have committed one or more clinical errors in the diagnosis, treatment planning or treatment of patients in the previous 6-12 months. Respondents would be asked to describe the error/s and how they were remedied, if they sought a second opinion or advice from their professional indemnity provider, state the number of years experience post-qualification they have and the BOS group they belong to. I have done a fairly wide search of the literature but could find only one article (from 2021) related to the topic of errors in dentistry. I would need help with ethical approval through IRAS.
