PDF Property Inspection Report Creation

We have a daily inspection forms currently in word. They are for our consultants when there on sites.
The basic form has job#, address , and a drop down menu that has:
- Asbestos fibre monitoring
- Lead monitoring
- Bulk Sampling
- Fire inspections
There are 12 different category's, when you use the drop down menu it automatically populates the template that is required for each task
Ideal Skills and Experience:

- Proficiency in PDF creation and formatting,
- Programming a PDF document with scipt
