Need a re-usable report for DWI case analyses

I am looking for someone to create and design a re-usable report template for my DWI case analyses. The report will need to include sections for phases of DWI investigations with some parts in check box lists. I have some general ideas about the format and structure of the report, but I am open to suggestions.
