Excel Regression on Patient Records

I have an anonymised spreadsheet of patient records and need a straightforward statistical check-up. Please run a basic regression analysis in Excel—using the Data Analysis ToolPak or equivalent formulas—to explore relationships between the variables I will flag.
What I expect:
• Cleanly organised workbook with separate tabs for raw data, regression output and charts (if generated).
• Key statistics clearly labelled: coefficients, p-values, R² and standard error.
• A short, plain-language summary (one paragraph is fine) outlining the main insight and any obvious limitations.
No deep modelling or elaborate visualisations are required; a concise, accurate regression report will do the job.
