We're building an AI-powered clinical decision support tool for anesthesia professionals. We need an experienced full-stack developer to create a demo-ready voice interface for our emergency protocol system.
Current State:
- React PWA frontend with JWT authentication
- OpenWebUI integration (AI chat interface)
- AWS infrastructure (ECS, RDS, S3)
- Dify AI workflow orchestration
What's Broken/Missing:
- Chat history retrieval needs fixing
- Voice interface (speech-to-text and text-to-speech)
- Backend proxy for HIPAA compliance
- Production deployment readiness
- Comprehensive documentation
Your Mission
Build a simulation-ready malignant hyperthermia protocol experience where:
1. Voice-First Interaction: Users can speak naturally to initiate and navigate clinical protocols
2. Collaborative Guidance: System provides step-by-step protocol guidance with closed-loop confirmations
3. Clinical Safety: Weight-based drug dosing calculations with verbal confirmation required
4. Fallback Resilience: Automatic switch to text if voice fails, resuming from last checkpoint
5. Clinical Export:*Session transcripts exportable as PDF/CSV for medical documentation
6. Sub-3-Second Latency: Voice interaction loop completes in under 3 seconds
Technical Deliverables (4 Phases)
Phase 1: Foundation (Weeks 1-2) - 60 hours
- Set up staging environment (staging.primaxis.health)
- Build HIPAA-compliant Node.js backend proxy (prevents API key exposure)
- Fix chat history retrieval bug
- Integrate Dify production workflow connection
- Documentation: Network architecture diagram
Phase 2: Voice Pipeline (Weeks 3-6) - 90 hours
- Speech-to-Text: Push-to-talk interface → AWS Transcribe Medical
- Text-to-Speech: AWS Polly Neural voices with playback controls
- Voice Commands: Finite vocabulary for protocol navigation
- Safety UX: Closed-loop confirmations for critical actions
- Mobile-responsive (iOS Safari, Android Chrome)
- Latency optimization to achieve <3s target
Phase 3: Clinical Workflow (Week 7) - 45 hours
- Integrate malignant hyperthermia protocol content
- Implement weight-based drug dosing calculator

Must Have:
- 3+ years React development experience
- Strong AWS experience (ECS, RDS, S3, Lambda preferred)

- Docker and CI/CD pipeline knowledge
- Clear communication and proactive blocker flagging
Nice to Have:
- Healthcare/HIPAA compliance experience
- AI/LLM integration experience (OpenAI, Claude, Dify)

- PostgreSQL optimization
- WebRTC or real-time audio streaming
Work Style:
- Comfortable with 25-30 hour/week commitment

Payment Structure Options:
1. Milestone-based: 10% upfront, 25% per phase (4 phases), 15% final
2. Hourly: $35-45/hour via Freelancer.com, weekly invoices
AWS Costs: Estimated $100-200/month additional (billed separately to us)

- Demo Milestone: Late December 2025 (date TBD)
- Final Delivery: January 13, 2026
