Except for the clusters and the symptoms, I'm open to your input on how to improve this from a UX or cost perspective.

Create elegant webpage where potential patients will
fill in "yes" or "no" (for potential patients), for each of the 13 clusters in here https://www.louisecarder.com/cirs "The 37 Most Frequent Symptoms Seen in CIRS-WDB".
Should be 2 options next to each 37 symptom,
1. radio button for 'Y/N', and
2. another a free text box for patient to add context, eg against headaches: "I have migraines at least a few times a month, which normally take me out for 1 to 2 days". At the end of the quiz, webpage: should show cluster in yellow where there's any positive responses; so patient (and practitioner) can easily scroll through and see where positive responses have been given.
When each of the 13 sections is completed, webpage should scroll down to next section and grey out or yellow previous section. Back button should be included.
No compulsory fields (I'm assuming)
Only thing that is compulsory is next button:
"Confirm no symptoms in last 6 months" OR, if any symptoms = Y, "Confirm symptom(s) in last 6 months".
Last steps should confirm submit, confirm email, then email them 2 page PDF of results: 1st page 13clusters/37 symptoms with yellow clusters and BOLD against Y symptoms, 2nd page with Y symptoms only with any user text.
The above is Stage 1.
Once completed, Stage 2 will be for existing customers to complete each month, with Y/N radio button replaced with 0 to 3.
Assume Login will be linked to one of the standard online HIPAA-compliant CRM.
