QR code development for COVID test

Title says it all. I just need someone to help me generate a QR code for a COVID test. I want something extremely simple. THIS PROJECT IS NOT FOR THE WHOLE DEVELOPMENT, this posting is only for the prototype.

Just to demonstrate that you can show me how it works if you develop one.
