Toogoom RSL QR Code

we need simple club QR Code
We need QR code so when members arrive they scan QR Code.
When they scan QR Code if they are pre registered they receive - Welcome back (persons name)
When they get welcome back I need option 1-Social Visit - 2-Volunteering - 3-Maintenance
If they are new visitor they receive welcome to Toogoom Sub Branch - they get option to register o in future they get the Welcome back (name)
We need the information of who visited the club, time of visit available online so we can use information and check number of visitors and all details.
