Custom QR Creation for 300 codes

We need urgently someone that helps us to create 300 Custom QR as image attached. We need the same colors degraded, the same logo size, the same type of QR. We need an image for every code, and the name of the image has to be the code. For example, if the code of a QR is 10157, the QR image name should be 10157.png or jpeg.
