create a python flask small app

attached is am form picture I need to implement in flask
it is an import from excel form

n a file there are header column names
each has its corresponding name in DB (not the same not in the same order)
also there is type of field
a file is in json format

has new field name , old field name ,type
on "import" write to afile and updated "the bottom fields
