Write a python script to transfer data from PDF to CSV

I have this document attached below.
All I want is a python script that writes the data from the document into a CSV with the following format:
Question | answer1 | answer2 | answer3 | answer4 | category_name | correct_answer (which will be blank)
This task is pretty straightforward as the document is very easy to navigate and has the same structure everywhere.
I am looking for a freelancer who will do this with as little cost as possible.
