Static Web App Redesign in Python

First of! Please don’t waste my time! I only need a frontend redesign. This is a simple application. Takes user input and should convert user input into a csv or could use a db as an option!
Data displayed to user in a form/template. Nothing fancy.

Don’t ask me what’s my max budget! My budget os $250 could be stretched a little. But you send me your price.
