Start up agreement

I need you to redesign my program
1. ones the user agree it should send the email that user is agree
2. they should select both agree then the accept is going to work.
3. the check mark only going to work when the user read complete the term and condition
4. When decline i need the user profile to be deleted
5. change the design base on the layout
6. i need back ground image for each term and condition

there is code done by other freelancer base on this requirement but im not able to verify
