Simple flask web page for python code

I have a python file to plot a graph for RGB values(will provide code or you can do it using pandas and matplotlib). I need a webpage which has 2 choices upload file and values. When values are entered the python file need to run if upload file is selected run another block. Title for the page needs to be " Lab values plot". Deploy using heroku or any open source.
