Simple Website with Python Backend

I need you to help me to:
- clean my python code and adapt it to be used not in a local pc but for a website backend (e.g. for Flask),
- develop a simple website around it and
- deploy it to the internet.

Website consists on one page (plus terms and conditions) where:
- 5 inputs fields from user (see file attached)
- Once button is clicked and the backend will generate a file and sent it to the provided email.

I want the website to be responsive, please.
I will deploy it on a server of a well known provided I may choose (probably namecheap one)

If success, more steps will come.
