Python simple webapp

Hello,
I need a simple webapp with these features.
1) This repo (https://github.com/CompVis/stable-diffusion) installed
2) The attached frontend template with a form for collect data
3) a MySQL that saves all rows connected.
The flow:
1) The user visits the frontend via url domain
2) The information filled in the form are saved into MySQL - with an ID
3) The repo reference runs with collected data from the MySQL
4) The image output gets saved into a folder and named as {ID}.jpeg
