Python selenium automation / scrape

Hi I need a site automated with search and the results scraped.
Attached is the input file. Need to search "https://www.legacy.com/search"
From the file you should be able to get first and last name, zip code, state and city. Once those are inputted on the site if there are matches we need to grab those URLs and put them next to the name in the output file (CSV format)
*This needs to be done in python*
