Python (ML/AI) and Database Work

Hello, I am looking for someone expert in Python (ML/AI) and Database. My project is to compare two Neo4j databases that represent the relationship between chronic diseases and patients. Hence, the goal of the project is to determine if the same entities are represented in both databases and use this information to build a reasoning system to infer new information.
I want to how to represent the real entities in a system (mapping) and how to write this reasoning system within a system as well. For example, the patient and drug will represent in ERD as entities and then will convert to the table in the database. the relation between patient and drug represents by a line between patient and drug entities in ERD then convert to a relation in the database
And in logic, we can say these systems have different syntax but they have semantically equivalent (the same meaning)
You can use a dataset related to the patient and divide it into two systems. The size of the dataset should be reasonable. I want to use logical rules for first-order logic or modal logic and I expect the outcome from the reasoning system of new information that does not exist in both systems or combines them to become new information. You can store data in a graph database or MySQL or any other based on your preference. I hope to store them in a graph database to show visualize my work. If you can support this work with an algorithm it will be good.
However, I do not have exactly any dataset of patients but you can find any dataset that includes healthcare related to two or more chronic diseases or you can assume the system and their data.
