Program to pull info from pdf and put into a webform

I have a pdf form and would like a program that could take the data from the form and put it into an online webform. The program would be made using python and java. An example of what I am looking for can be found in the video below. I attempted this project myself but cannot identify the ID tags in the online webform in order match them up with the pdf. If someone can make this program for me, or write another program that would pull the data from the pdf and put it into a webform, that would be great.
