Need to scrap a single page in Python using Selenium and Beautiful Soup

Hello, A Python scraping expert is required to scrap a single page in Python using Selenium, chrome webdriver and Beautiful Soup. Please have a look at the attached screenshot of the page that is needed to be scrapped. The scraped data needs to be inserted in the SQL Server table using the same column name as given on the page.
I'll also provide a sample code that was written for an exactly similar project.
