Modbus Sensor Data Integration Python Script

I need a Python program to monitor temperature and humidity readings from a Modbus sensor (THD-D1-T) connected via Moxa (NPort52321) over a TCP/IP connection. The script should:
- Connect to the sensor via the Moxa server.
- Read the sensor information every hour.
