Convert Polish Text to International Phonetic Alphabeth (IPA)

I am looking for a porson that will write a code (or do it manually) that will convert Polish Text to International Phonetic Alphabeth (IPA) so that it fist exactly that format: https://raw.githubusercontent.com/menelik3/cmudict-ipa/master/cmudict-0.7b-ipa.txt

The text to be converted is attached.
