Configure Rasa Chatbot showing some hierarchal options/selection stories and Forms

Hi, I have started a Rasa project for a chatbot config (using the Rasa open source), configured some of the options but would like the Rasa expert to complete a sample showing multi option hierarchy stories and forms as per attached file. Thanks
Note that I'm not asking to build the widget. All what I'm asking for is to config/build the rules/stories/domain Yaml to reflect the scenarios. You can test the configured results based on an existing widget mentioned in my document.
I will provide the YAML config files I have if required for the selected contractor.
