Proofreading , editing , formatting manuscript

I have a manuscript with 35% plagairism content. I need to reduce the plagairism and require an excellent English language writer to do proofreading and improve the language in the manuscript and adjust the formatting of the manuscript to a journal format attached. Also I require to create professional table of contents and table of figures and to hyperlink it with the manuscript.
