Registration Kiosk
Budget: $750 – $1,500 USD
I would like to create the following program for my practice:
1. A program that enables patients to fill out a questionnaire where they will provide personal information. The website or program should be useable on their cellphones, computers (Mac or PC), and tablets. Method of hosting (e.g., website) is at the discretion of the programmer.
2. The program will extract information from the questionnaire and populate it into the corresponding fields in two programs. I DO NOT have API's for these programs.
a. PM (Patient Management) - A financial program that stores demographic and financial information.
b. EMR (Electronic Medical Records) - A medical records program that stores medical information.
(We have attached videos of the manual input of information into these two programs).
The program will enroll NEW patients (NP) and update RETURNING Patients (EP).
NEW PATIENTS (NP)
- will receive a link that allows them to enter relevant demographic information. (See questionnaire)
- Each question has a corresponding field in PM and/or EMR. (Note that some fields are only in one of the programs, e.g., medical questions (see attachments) will be only uploaded into the corresponding fields in EMR.) (See video).
- The data provided from the questionnaire need to be 'typed' into the PM automatically by YOUR software.
• Once a new patient (NP) is created in PM, a “bridge” that brings the NP into EMR is activated by several key strokes. In this way the relevant data from PM is transferred into EMR.
ESTABLISHED PATIENTS (EP)
- already exist in PM and EMR.
- They will receive a questionnaire pre-POPULATED WITH THE EXISTING DATA FROM PM and EMR.
- The patient can review and update this information.
• If changes are made, the revised data will be uploaded into PM and EMR in the same manner as a NP is entered.
• If no changes are made, the patient will check a box- "no changes".
- A note will be made in a “sticky note” field of the EMR that records the date and that "patient reviewed all data".
OTHER FEATURES
- There will be LINKS for the patient (NP or EP) to upload their insurance cards (both sides) and their driver's license or photo ID) The upload should be made possible from a cellphone picture or scanned image.
- There will be FORMS that must be signed by the patient acknowledging certain terms or conditions.
• Some of the forms will be relevant to all patients.
• Others will only be presented based on information that is provided in the questionnaire (e.g., patients with Medicare insurance will receive the Medicare document, others will not)
- The program should be adjustable in order to allow me to
• add / delete/ or move fields in the questionnaire at any time in the future and to track them into corresponding fields in the PM and EMR.
• assign fields as required or not required at any time.
- ALL OF THE INFORMATION PROVIDED FROM PATIENTS NEEDS TO BE STORED IN A DATABASE:
• The database must be searchable (e.g., Excel) and allow review, report generation and transfer of data at any time.
• Each entry by a patient must be sent to the office's email in a similar form in case a field is not properly populated or the data does not integrate properly.
• A PDF of the finished questionnaire must be sent to the patient email as confirmation of their enrollment
The Website/Program must be ROBUST and DURABLE. It cannot stop working if a field is omitted or entered improperly by the patient. It should alert the user to the error at the time and NOT halt action and require reentry of all data.
Time to completion and cost are relevant.
Please review the scope of the project and bid realistically. I will NOT start negotiations after your bid is submitted.
