Vet Clinic Flyer and Feedback Form Design

Hello,

I am looking for a talented graphic designer to create two visually appealing print materials for my veterinary clinic, Super Pet’s Hospital:
1. Promotional Flyer (Double-Sided)
Purpose: Promote the hospital’s services and build brand recognition among pet owners.
Design Style: Vibrant, clean, and modern. Should be attractive to pet lovers while maintaining a professional look.
Size: A5
Sides: Double-sided
Front Side: Logo, services, tagline, brand values
Back Side: Contact info, social media, address, any quick offers or highlights
Output: Print-ready file (PDF), image (JPG/PNG), and source file (AI/PSD/Canva)

2. Feedback Form (Single-Sided)
Purpose: Collect customer feedback and understand where clients are hearing about us.
Design Style: Soft, minimal, calming — easy to read and fill out.
Size: A4 or half-A4
Sides: Single-sided
Content Includes: Client info section (Name, Phone, Email, Pet’s Name)
Multiple-choice checkboxes (e.g. Instagram, Google, Referral, etc.)
Optional comment section. Footer with social media and thank you message

Output: Print-ready file (PDF), image, and editable source file

What I Will Provide: All written content, Brand logo ,Color preferences, Example designs I like (optional) tagged below-

Deliverables:
Print-ready PDF for both designs
High-res JPG/PNG
Editable source files (AI/PSD/Canva)
2–3 rounds of revisions if needed

Timeline:
