I have a clear photo of a two-page form, and I need that exact layout rebuilt as a print-ready PDF. The goal is to mirror the text, general layout, and structure you see in the image—spacing, alignment, columns, boxes, and any section breaks—so the finished document looks indistinguishable from the original when printed.
