Printable Product Survey Form Design

I already have the colour palette, watermark image and company logo ready; what I need now is a clean, print-ready survey form that customers can fill out by hand to give us product feedback. The core of the form will be multiple-choice questions, presented in a way that feels clear and uncluttered while still reflecting our brand style.

The finished piece must:
• Follow the supplied palette, watermark and logo placement guidelines without distortion.
• Fit on a single A4 page with balanced margins so it photocopies cleanly.
• Include space at the top for basic product identifiers (model, purchase date) and then flow directly into the multiple-choice section.
• Arrive as a high-resolution PDF (ready for print) plus the editable source file.

If you can also suggest minor typographic tweaks to maximise legibility when printed in bulk, that would be appreciated.
