PowerApp (mobile and web) for a railroad company to submit driver requests

I am looking for someone to create a PowerApp with features for a Vehicle Driver request submission form that includes the user’s driver information and should produce a PDF that can be used in an existing workflow at a railroad company. The PowerApp should be optimized for both mobile and web access. Additionally, I have general ideas for the design and layout of the project but am open to suggestions. Ultimately, the PowerApp needs to accommodate more than 50 users. It also needs to have a French translation. I am hoping to find an experienced freelancer who can help me develop this PowerApp and bring it to life. Thanks for considering my request.

The Powerapp needs the following fields:
Employee Name
Employee ID - ABC####
Requested By - ABC####

Driver's License Number
Province of Issue (drop down)


Then the resulting PDF that can be used in our existing workflow should have the following-

Employee Name
Employee ID
Requested By

Company
Division
Manager
Email
Alt Email
Driver's License Number
Province of Issue
