PowerAPP with field inspection application

Hello, I want to have a field control application made with powerapp. The application will consist of 4 types of users. Managers who will have a top manager of each user will be able to examine sub-managers. Users will take a photo of the problem they see in the positions assigned to them and write a note under it and send it to the top manager.
