Power Apps SharePoint Submission Form

I already have a SharePoint list in place; now I need a straightforward Power Apps canvas form wired to it so that every time a user clicks Submit, their entry is saved as a brand-new list item. The form only has to capture plain text and numbers—no file uploads or attachments. All fields should open empty by default, giving users a clean slate each time. It should have a drop-down which takes values from one of my SharePoint lists

Please connect the form to my existing list, map each field accurately, and make sure the update happens instantly with no manual refreshes.
