POC DataVerse with Model Drive App

I need to create simple POC application in DataVerse and the App Must by created in Model Driven App...
Two tables

Table 1- RequestForLeave
Columns
User - User Picker from Table Users prefiled by Current User
User Name - Single Line Text prefiled Current User
User Number - Single Line Text prefiled Current User
Department - Single Line Text prefiled Current User
Remaining Days - Number
Substitute - User Picker
SuperVisor- User Picker

Table 2 - Section
Columns
Allocation - Choice
From - Date
To - Date
Days - Number

RequestForLeave 1 -> * Section

In Main Form RequestForLeave must be a repeating table for Section...There must be a relationship so I can see only entries for specific row in table RequestForLeave.

This is only POC...I need to see your work on this POC and then we can continue...
I must be able to add entries, edit and Delete...Simple standard application
