Kobo tools form creation

I need to create a form in kobo tools. i have the format ready.& not too much of the work is required.



Detailed form layout:



New

Existing



If new selected -> Name|Num|Address|Beat selection (This comes frōm an external CSV) - close



If Existing selected-> 3 cascading options shud appear (Options frōm an external CSV)

For example: Chennai-> Avadi -> MG RŌad.. So when MG road is selected, pull data function comes in and displays (name|Number)
