Checklist App for Resturant

Create a checklist Power Apps
1) The app should be responsive
2) Checklist should be backed by Excel
3) Should have a train like feature (Start -> Check list for Front. Office -> Check list for Back Office)
4) Start would have basic details (Branch, Branch ID)
5) The checklist should be populated. from a list stored in Excel
6) While saving the data (Created by and Date) should be auto saved
7) Dashboard to show the details based on filters - I can send an sample
8) Checklist should have Pass & Fail and for Failed list user should be able to add comments
9) Show summary before form is submitted
