Build a flow using existing form using power automate

Hello,

I have a microsoft form for covid screening.
I want someone to. use power automate to use this form (link to duplicate is below).
there are 7 questions, all are required. If any user answers any question as yes it should send and email to me.
if they answer all questions as no , it should say you are ok to go in office.
