Add Notes Change Logging

I want to extend my existing Power app so that every edit made to the main Note field that appears on the UI is captured in an audit trail. I already built the same feature in another, almost-identical app and will give you full access to that code so you can transplant the logic rather than starting from scratch.
The behavior I need is simple: whenever a user makes a change whether it be the admin rejection or a hold or the end user, the app should automatically record the change, the editor’s user ID and a timestamp.
You will deliver an updated version of the app and a short note explaining where you placed the new code and how I can adjust it if the Note field name changes in the future. Once I import your file and see the log filling as I edit that field, the job is done.
