Microsoft Forms Approval Workflow
Budget: $30 – $250 USD
I need an online form built in Microsoft Forms that captures Name & contact details, Department & manager details, and Reason for approval. When someone submits the form, a multi-step approval flow must start in Power Automate, automatically selecting approvers from the submitter’s manager hierarchy.
The flow should:
• Trigger an Outlook email and a Microsoft Teams notification at each approval stage, showing the form data and action buttons.
• Escalate or remind approvers if they do not respond within a set time (please suggest a reasonable timeout).
• Log the final decision and any comments back to an easily readable SharePoint list or Excel file for reporting.
Deliverables:
1. Microsoft Form ready to publish.
2. Fully configured Power Automate flow with the steps above, tested end-to-end.
3. Basic documentation (screenshots or short markdown file) so I can maintain the flow later.
