A web application for vehicle movement control

We are a transport company having branches all across the nation.
We require a web application for vehicle movement control which will be hosted on the private network of our company (not on the internet). The description of the application required is as follows.
A web application having 5 levels of authority to sanction movement of vehicles.
The initiator (level 5) must be able to fill in an application requiring sanction for its movement of vehicles {Format of application will be given}.
The application reaches authority at level 4 which may sanction / reject the movement of vehicles.
The level 4 authority may recommend and forward the application to the next authority at level 3 if the movement of vehicles is outside it's designated area.
Similarly, authority at level 3 may sanction / reject or recommend and forward the application to next authority at level 2 if movement is outside the designated area of level 3 authority.
The last sanctioning authority will be at level 1 if move is outside the designated area of level 2 authority.
The application once sanctioned (at any level) will be reverted back to the initiator who should be able to print it and should be able to track the status of application when it is forwarded to next authority.
Columns stating sanctioned, rejected and forwarded applications should be visible on the web application.
