turn excel sheet to online form
Budget: €30 – €250 EUR
I am looking for someone who can help me turn my excel spreadsheet into an online form. The form should have basic form fields such as text boxes and checkboxes. Ideally, I would like the form to have advanced fields such and conditional logic.
The calculations must run "live" so when you change a field the form should update.
The form has to be dynamic - so that user can add additional lines and so on
This is a startup project, so more work can come. This is only a small start mockup but if everything goes as planned the project will be large scale.
The files should be with lots af commentation. I understand PHP and HTML - but my javascript skills ar not so great.

It would be great if the form was built using php/javascript/HTML. I don't have a strict deadline for completion but the sooner the better. Thanks!
