simple database with added code.

i need to input 4 data inputs and have user check what data is there. match all 4 data correct and it shows points. any data out of the 4 incorrect = no user found. i should be able to add and modify all 4 boxes and amount of point box add and subtract. i have supplied code for 98% of the project needed. i need you to blend them together. also a $Tip to you personally if i can have this today!
