sign up form plus output and database

Hello! I'm looking for someone to build me a sign up form plus an output and database. The form should just request basic contact information from users. I'm not picky when it comes to the database system - I don't have any preference.
This is for a members sign up, I would like their details to be saved in a database, each member assigned a unique membership number and an automatic confirmation email sent to confirm their details, after acknowledging this confirmation email their number is provided and their membership confirmed.
And finally, I'd like the output to be in the form of a CSV file.
I have stripe, and am currently using Typeform for the sign up form but would like to drop that if possible.
