run query, combine info, and merge info PDF

I am looking for a web based program that can do the following. Needs to be php based and be able to run on Linux.
1.) accept user input
2.) run queries mysql (i'll provide)
3.) allow user to select data from query
4.) combine the data to a table format
5.) allow user to select images (jpg/bmp/png/pdf)
6.) combine everything into 1 single pdf
7.) allow user to write file back to server or download
