registration form dosnt work (JS, PHP, MySQL)

1) i cant register any new accounts, check and fix where problem
2) add to field password (eye icon, when click i can see password)
3) i need to add option under buttons (restore password - link), after click to link i can put adress email and script will send new randomly generated password
