I need to update my customer curriculum in internet real time

I need a google form or something similar where we can fill in the information of our customer project and 4 pictures, that can be uploades, and that information should go to a section of our website a Specific URL where the projects are going to be organiced by STATE of our country and at the same time the visitors can filter by state the projects...

In easy words we need to be able to upload data and photos, and they should be easily organised and filtered in a url of a website,
