I need someone to check scripting

Have a web contact form on web site which partly works, i.e. person filling in form receives an email copy but business doesn't receive a notification of form been fill out and submitted. The program used to create form is coffee cup form builder. One folder named Bryant's Motel Roma Contact Form 2 and one PHP file named Bryant's Motel Roma Contact Form 2.php are uploaded to domain - http://bryantsmotelroma.com.au - at (Crazy Domains) . This domain is a sub domain to http://mjkbusinessmobileapps.com.au which is the Linux web hosting domain. Form is on http://bryantsmotelroma.com.au/contact.html I have uploaded files for you below. Bryant's Motel Contact Form 2.php is a file uploaded by itself and all other files are uploaded in the Bryant's Motel Roma Contact Form 2 folder. You can see details in user.cfg.php server needs authorisation and is SSL/TLS. I think I have filled out correctly but may be the problem. Also a screen shot of MySQL details.
