I need to create a pretty simple interactive form. I think it's doable in MS word Developer mode but I'm not 100% sure

I am a physician working in a large group. We cover anywhere from 1-12 hospitals during the day. We have created a timeline so that people know what to do when, but we now use 2 physicians during the day. Based on where they are licensed and credentialed, they cover anywhere from 2-10 of the hospitals. I would like them to be able to select which hospitals they are covering and then have the document (a to do list) populated in chronological order based on their selection. I have attached a sample timeline that we use. This timeline is outdated so I still need to edit it. I would like not only an interactive document to be created, but also I need to have the ability to edit it (change the task/time for each hospital or add new hospitals all together). If this is not doable in MS Word it could be done in forms or another MS app. It just needs to be a MS product or a product that is designed for use with MS.
