frontend registration portal needed

Please we are currently using a WordPress form to accept registrations, but there are a lot of customer complains, so we have decided to switch it to Laravel, and a well designed and easy form, please note that we will want the form and submitted to database and a copy sent to 2 email address, the form is about 5 pages, it must be 100% mobile friendly and must allow for resumption if not completed before, there should also be phone and email verification before form is submitted.
i need a very good theme for this
