develop wordpress plugin for macronutrient tracking

we need a wordpress plugin for weight and macro tracking (like this one https://fr.wordpress.org/plugins/weight-loss-tracker/)
When the user submits initial form, it calculates his weight loss plan and he receives the results in a PDF by mail (you will find the content of the results in the page 6 of the Formulas document).
We also need the data to be stored in wordpress database so the user can track the evolution of his weight loss (so we will also need to display users results).
