I want a customer survey web application to be built using PHP and MySQL only (no other languages, sorry)
The application has around
- 50 front-end screens
- 30 back-end MySQL data tables
We will provide the winning bidder with webspace for hosting the code files, and My SQL space for the databases

What we are looking for - Your ideal skills and experience:
- Strong experience in PHP coding and building customer-facing applications
- Excellent attention to detail
- Ability to work efficiently and meet strict deadlines
Project Scope:
- All screens described in the project brief should be accurate
- The project needs to be completed within the timeline.
Deliverables:
- A customer survey application which run on the web (we are not looking for a mobile app)
