Wordpress themes and php expert needed

Hello,
I need Wordpress themes and php expert developer needed to modify some signup form of wordpress theme the scenario is , when a user try to signup using theme builtin form its require those fields :
1- First Name
2- Last Name
3- Phone Number
4- Email Address
5- Password
---------------------------
I need to add more fields to this signup form like :
1- Work Title (Text Input Field) - already have some value or strings in functions and hook file of the theme
2- Select location (Dropdown with select option) code is exist and - already have some value or strings in functions and hook file of the theme
3- another field for password to repeat password again
4- Text box so users can write description about their membership - already have some value or strings in functions and hook file of the theme
5- Add more field also have some values in theme functions

all required fields have already working in dashboard of the member and its editable up there but i need to copy this forms to front-end signup form to be filled pre signup and store the values into the database correctly as it types from dashboard .

there is also two radio options as two memberships types i need to hide those extra fields for one of those member types radio button if selected.
